Application and Recommendation Forms

In order to meet the requirements for admission into the Team Leadership program,
a prospective participant must:

  • Be at least 17 years of age
  • Be a current high school or college student in good standing
  • Be medically, physically and emotionally stable
  • Be willing to participate fully and comply with program expectations
  • Have a serious interest in helping their peers transform their lives
  • Have a minimum reading and comprehension level of grade 11

Due to extensive preparation that will be conducted before the start of the program, perspective participants must submit their application and recommendation forms no later than three weeks prior to the start of the program.
